Start your performer application

Business name under which you operate (no LLC or Inc allowed at end of DBA, as this is a policy for an individual person or sole proprietor). If the DBA listed is a larger organization (like a circus or event planning company, for instance), the performer’s name must be added to it. Example: instead of “Extraordinary Productions” as a DBA, it will read “John Smith of Extraordinary Productions.”
Provide a password for the new account in both fields.
